Immediate implants in a nutshell: biology, technique and devices

In modern dentistry, the use of dental implants is considered to be the standard of treatment for edentu- lism. The original protocol for the placement of dental implants required a healing period of 6 to 8 months after tooth extraction, plus a stress-free healing period of 3 to 6 months for osseointegration. In order to meet the the esthetic expectations of ever-more demanding patients, immediate dental implant placement was introduced, and is today widely recognized as a very reliable technique. Even if esthetic appearance is receiving more attention from surgeons, clinicians continue to debate whether the immediate implant placement technique can completely preserve primitive volume of the alveolar crest, or prevent alterations of the buccal and lingual walls, both in terms of height and width. Nowadays even if we know the exact cause of bone loss after tooth extraction clinicians and researchers still debate about the role of several factors, such as the socket location, the thickness of the buccal bone plate, the insertion torque value, the marginal gap between the implant and the alveolar bone wall, the implant collar design, the implant surface nanotopography, and the vertical location of the implant-abutment connection, and the residual bone height. It is evident that immediate implant placement requires adequate experience even in simple cases, due to the large number of factors playing a role in the esthetic outcome of post extractive implants. The aim of this seminar is to present our experiences in terms of immediate implants and to discuss about the latest acquisitions in terms of post-extraction socket management.

Winner of the ADR/PHILIPS ORAL HEALTHCARE YOUNG INVESTIGATOR RESEARCH GRANT with the project titled “The influence of different surgical protocols on the outcome of dental implants in diabetic patients.”. Author of more than 50 scientific publications in impacted scientific journals (see below), co- author of three books, active member of IADR, IAO (Italian Academy of Osteointegration).
